From the Sewers: by using the stairs down at {03,29,00}, or by falling through the pits at at {3,21,0}, {03,27,03} or {03,27,09} (to reveal a treasure), or by unlocking the door upstairs at {03,12,00}.
From the Dwarven Maze: The wall and the door can be opened from there.

By falling down the pits at {03,27,03} and {03,27,09}, you get a rapier, scroll "SEND RUSTED AND BROKEN TOOLS TO THE FORGE FOR MENDING.", 2 iron keys, a Gold Key, an Emerald Key
Shuriken: open the door at {04,27,11} (switch nearby)
"Inflamables no Plasmas": the door can be destroyed with fireballs. You find Blast Spores behind it.
+1 Competition point: Funereal Dagger at {04,23,09} (bashable door or Iron Key lock)
Stormbringer at {04,23,04} (bashable door)
Scroll "STOP YOUR FALL WITH A MAGICAL NONWEIGHT" (This seems to be referring to a Plasma, but what does it mean?)
The scroll "SEND RUSTED AND BROKEN TOOLS TO THE FORGE FOR MENDING." refers to the repair alcove in the Dwarven Forge. Put items marked as "BROKEN" into it. Rusters can break metal items, some items are found already broken.

To the Officers Canteen: teleporter behind a door with an Iron Key lock
